Fill in information as requested. If you need more names use the button "Add 10 Rows", as needed. Please read the instructions given on the page. Note that there are more detailed instructions you may wish to read/review at the link below:

Instructions to fill out this form. If you are familiar with the Court Report form, you may still wish to review the instructions ...

Information About the Court:

(Required) This is very important: WHO GAVE THE AWARD(S) LISTED? The selection here determines which Royalty and which Regional Herald this report is sent to. Please make sure to select the correct Region the Royalty are from (includes Barons and Baronesses)!

The listing here shows the royalty of the region selected in reverse (descending) order of reign, so that the most recent is the first one shown. Note that some royalty may appear more than once in the list, in that case it doesn't matter which you select (ex. Jade and Shaheena will be in the list for the Kingdom three times). Please double-check this, if selecting the "Royalty of" did not update this list, click the small blue "refresh" button to the right.

Alternate Royalty Addresses are Optional, but important: There are cases where the default email address for the Royalty may not work (such as the royalty this report is for are no longer on the Thrones ..., etc.). If you have an alternate email address for the Royalty this court is for, please use it here. May also be used for head of court ... Please fill this out if the report is for the outgoing Royalty after a Coronation or Investiture event. This will mean making sure you have the email addresses for the outgoing royalty.
(If you need to use a second address, please add to the appropriate entry area above.)

(Required) (MonthName, Day Year
[i.e., March 10, 2015], please!)
PLEASE DOUBLE-CHECK THE DATE! This defaults to the current date, not the event date.

(Required) Roman Numerals, ex: LII -- note that this is filled in for you by code in this form.

(Required) Unless this is a Kingdom event, specify region (i.e., Mists Investiture, Eskalya Yule, etc.).

Optional, this is useful for an event where there are multiple courts, such as a Crown Tournament, noting "Morning Court", or "Evening Court", or at a Coronation, "Coronation Court", etc.)

(Required) The name of the Court Herald (not the person submitting the report unless they are the same person).

(Required) The email address of the court herald. Please make sure this is a valid email address.

If the court report is being submitted by a different herald than the court herald (which is perfectly acceptable), please enter your name here.

As with the submitting herald name, if the report is being submitted by someone other than the court herald, enter a valid email address here for the submitting herald, so that you can receive a copy of the report.

Herald -- PLEASE READ:

Specific instructions/requests:

  1. Please double-check spelling of names, you may wish to visit the Awards List (Name Search) site or the West Kingdom History site's Who's Who to verify the currently-used-spelling of someone's name if they are in the list.
  2. Please group awards of the same type together -- for example, if there were six Awards of Arms, please go through your report and put all six of them one after the other in the list. (This assists the Chronicler in getting the awards listed in The Page properly.)

If you have lists of one type of award (consider two or more to be a list), such as Queen's Cyphers, please do not repeat the name of the award for each entry. Put the name of the award for the FIRST recipient, and then for the others just put the names in the second column. Once you have finished with that specific award type, then put the next award name in, and continue. For example:

Award(s) / Honor(s) Given     (Recipient) SCA Name Scroll? New to
    Awards List?
1   Leaf of Merit    Alfred the Smart
    2      Bjorn Bearsson
    3      Fred the Lame
4  Award of Arms     Albert the Not-too-Smart
    5      George the Tall
    6      Sheena Red-hair
    (and so on ...)

  3. Please alphabetize lists of multiple awards (any/all) -- it is easier on the reader when a list like this is published to find an individual name ...
  4. If the award is numbered (such as the Cynaguan Ruxton, the Pillar of the West, etc.) please include the number for that recipient. See: Regional Notes
  5. "Scroll?" Checkbox -- if a scroll was given in court, the herald should be aware of it, and should note it on the court business, so that when filling out this form, you can simply check it. If you don't, it won't be a problem, but it is helpful.
  6. "New to Awards List?" Checkbox -- if this person does not appear in the Awards List, please check this box. When the report is processed a note will be added for the individual for the Golem Herald. If you have more information about this individual, please add it to the "Notes:" section below, but if all you know is that you could not find them in the awards data, please be sure to check the box here.
  7. If multiple forms are used please use the "Notes:" section below to state "This is part n of this report" where "n" is a numeric value, such as "This is part 1 of this report".
  8. If reporting reaffirmations of awards note with the award the Royalty who gave the award originally, and the date (include SCA Year). Example (place this in the "Award(s) /Honor(s) Given" column!):
Reaffirmation: Award of Arms, Fred and Wilma, King and Queen, 06/01/2005 (AS XL).
Don't worry about the length of the text - it will fit, although it doesn't look like it (see image below).

    Example Reaffirmation
Note that information about Reaffirmations can be gotten here:  Reaffirmations of Awards
  9. Regional Notes: Some 'regions' have specific awards you should be aware of for reporting information, you may want to check this page: Regional Notes.
  10. Notes: If a person is being added to the awards list for the first time (in other words the submitting herald cannot find them in the awards list), please see the "New to Awards List?" checkbox above, and if you know more (modern name, gender preferences, location -- what their local branch is) please make a note. Make notes about other concerns as needed ... More information is better than less. If there is anything you aren't sure about (officer changes, and so on) please use the Notes field.

The Following Honors Were Given (this page defaults to 30):

Please do not leave any blank lines between names of recipients -- doing so will end up not reporting anything after the blank line ... you can leave out the award name for multiple recipients of the same award, but once a blank Recipient is found, the program will stop.

This button will add ten rows to the table above each time it is clicked. This is useful for courts with long lists.

IF you have a very large report with 100 or more recipients for a single award, you are permitted to attach a Word Document. Please sort the list of names in Word. If you are using something like LibreOffice, please be sure to save the document as a Word Document. If you abuse this the court report will be returned and you will be asked to fill in the standard report above. If you attach a file you must fill in the the "Award(s) / Honor(s) Given" in the first row above (use the name of the award), and then in the "(Recipient) SCA Name" column for the first row add text like See Attached File. If you do not fill these in the form will kick you back to those entry areas and require the information for the first row.

If an individual is not currently in the awards list, please check the "New to Awards List?" checkbox. If you have the following information (and you are encouraged to please try to obtain it), put it in the Notes above:

  • Gender (some names are not obvious) which can include "Male", "Female", "Non-Binary/Other", "Unknown"
  • Modern Name (if known)
  • Kingdom (if they are not in this Kingdom)
  • Local Branch (if they are in this Kingdom)

If there are concerns, items you aren't sure should be in the list, whatever, the more information you provide the better. Questions can be left here if needed, etc. Please do not use a web address (URL) -- it will be rejected and the court report may need to be filled out again ... this is due to spambots and bad behavior websites and such. Included in "do not use" are copies of data from spreadsheets, or information directly out of any Microsoft office software (Outlook, Word, Excel, etc.) -- these can cause serious issues in formatting and sometimes cause the "send" to fail, due to the XML used internally to format the information.

Please check your entries, check the "This Form is Complete" checkbox, and then press the "Send Report" button below to send it.

Reminder: Before you submit this, please check the royalty and date for the court that these awards are given. The date component above defaults to today's date, and there have been a few instances where the wrong royalty came through, even though the submitter was sure they had selected the correct royalty.

If you have more than one report for the same event, after using the "Send Report" button, on the new screen that shows your report as it was sent, there is a button "New Report for Same Event", or you can use "New Event" if you need to start a report for a different event. Using "New Report for Same Event" will fill in the information at the top of the page, and should be updated as needed (date, different royalty, 'Court Description').

A copy of this will go to the current Royalty for the region selected, to the Golem Herald, Senior Herald (Vesper/Green Crown/Principality, etc.), West Kingdom Chronicler, and to yourself (so you may have a copy for your records), as well as any other addresses given.

NOTE: You must click the checkbox below to enable the pushbuttons.

Please do not click the "Send Report" button more than once ...
