Please give the SCA Name as it currently appears in the award list, explain changes in "Notes" ... otherwise it will be hard to find it in the database.

Optional: The modern name is helpful for the administrator to look up a person if the SCA name is not known, or not remembered. This is not displayed anywhere on the site except for the administrator.

Enter the name of the award/recognition here. If you are dealing with multiple awards add the others to the Notes/Comments/Suggestions field below (please provide all the detail requested here). See note at top of screen if you moved here from another Kingdom.

Month day, Year format (i.e., April 22, 2001) if known.

Region of Royalty who gave award, if King and Queen, select 'West (Kingdom)', if Prince and Princess, select name of Principality, etc.

NOTE: If you received the award in another Kingdom, please enter the name of the Kingdom/Principality/Barony in the 'Royalty' field, not the names of the Royalty who gave it -- we don't track that information. (There are too many options for this to come up with a coded list ...)
