If you are requesting information about an award scroll, please visit the West Kingdom Company of Scriveners and Limners, as the scribes are responsible for producing and tracking award scrolls.

SCA Name:

If your name is registered, please use your registered name, as that is what is in the award list ...

Recognition (Name of Award or Honor):

Please use the name of the recognition, preferably as it is used in the awards list, it makes it easier for us to find what is needed.

Date of Award:

Modern date: Month day, Year (i.e., April 22, 2015)

Event Award Was Given At:

Give the title of the event, and the local branch where it was held.

Region:

Region of Royalty who gave award -- this will determine the email addresses to send this request to ... in some cases it will be sent to the current royalty of the region, in others to a regalia officer (if there is one).

Royalty:

Name(s) of Royalty who gave the Recognition: Sovereign and Consort, even if the recognition was only given by one or the other (for example, if you were given the Queen's Order of Grace, while given by the Queen, please give both the King and Queen's names).

Your Email:

If you do not provide a valid email address, the royalty and/or the regalia minister cannot contact you if they have any questions.

Notes/Comments/Explanations:

Please provide any information you think will help ... if you wish the token mailed to you, please provide appropriate mailing information (address, city, state, postal code). If you are missing multiple award tokens from the same region, list each with the details as shown above (award name, event, date, and royalty ...).
