A Reaffirmation of an Award is for an award that did not get reported properly and/or did not appear in the Awards List in a timely fashion. This is a request for the current royalty to affirm that the award was given, by whom, and the date it was given. This will get the information into the proper channel for processing and ultimately into the awards list data. More explanation of what this means can be found at: Reaffirmation Of Awards Explained.

Enter as much of the appropriate information below as you can, and click the Submit button to send to the appropriate Royalty, for reaffirmation of an award. Please Note: It is alright for you to fill this in for someone else if they are uncomfortable with doing so. You may want to, in that case, use your email address instead of the recipient's, so that if the Royalty have a question, you will get the email asking for clarification.

NOTE: All of these items except for the OTHER NAME and NOTE fields are required -- if left blank this form will not be sent (you will see an error page explaining the problem) ...

SCA Name:

Please enter your SCA name as it is displayed in the Award List (if you are submitting this for another individual, please use THEIR name as it is displayed in the Award List).

Other Name:

If the award was received under a name other than your currently registered name, or may have been under that name, please provide that information, if you have received recognitions under more than one name, please provide them all.

Award (Name of Award, Recognition, or Honor):

Please enter the name of the award you are requesting be reaffirmed.

Event Recognition Was Given At:

Give the title of the event, and the local branch where it was held. (i.e., Madness (Westermark)) If you cannot remember, please state so [I cannot remember].

Date of Recognition:

Month day, Year (i.e., April 22, 2001)

Royalty of:

When selecting, this is about the royalty who gave you the award:

If the award came from the King and/or Queen, select "The West, Kingdom of"
If the award came from the Prince and/or Princess, select the name of that Principality
If the award came from the Baron and/or Baroness, select the name of that Barony

This is very important or the request will go to the wrong people!

Royalty:

Names of Royalty who gave the award (Sovereign and Consort), even if the award was from one of them, it helps to narrow things down. If you received a Queen's Order of Grace, please note both the King and Queen ... (If you misspell a name, that's okay, we'll figure it out.)
